my son is 5 months old and sometimes his forehead seems to be hot but not every time should I worry about it

No need to worry, apply oil (coconut oil or sesame oil or castor oil) to head regularly.
Apply coconut oil to the body and leave for 15-20 minutes before bath or else do sponging , coconut oil light in texture absorbs easily , absorbs heat in body, reduces warmthness in body.
