The condition may be prevalent in children who are suffering from congenital heart diseases, are born with narrow hearts or any other heart conditions. Types of Arrhythmia
Arrhythmia is categorised based on the variation in the heart rate. There are many types of arrhythmia; most are harmless, but some may be life-threatening.
If the kids heart rate speeds up and becomes too fast, it is known as Tachycardia. If it slows down considerably, it is called Bradycardia. Both conditions are explained in detail below:
1. Tachycardia
Tachycardia is characterised by a faster than normal heart rate and may be caused due to an underlying heart condition, medications or some changes in the physiology. It doesn’t last long enough to cause any serious damage to the organs. There are two significant types of tachycardia.
Ventricular Tachycardia
Ventricular tachycardia originates in the ventricles or the lower chambers of the heart. These are not very common in children and might be the result of a serious heart condition.
Supraventricular Tachycardia (SVT)
Supraventricular tachycardia is triggered in both the upper chambers (atria) and the lower chambers (ventricles) of the heart. SVT causes the heart rate to go at an abnormal rate of over 220 per minute. It is also known as paroxysmal atrial tachycardia and begins and ends abruptly.
SVT may be caused due to Wolff-Parkinson-White Syndrome, where there is an extra pathway in the heart’s electrical system, which can result in tremors, dizziness, and problem breathing.
There are several types of SVT, which include the following:
Atrioventricular Reentry Tachycardia (AVRT) – AVRT is caused due to the presence of an additional pathway because of which the electrical signals do not pass through the AV node that controls heart rate. The heart rate increases as two pathways carry electrical signals at the same time.
Atrioventricular Nodal Reentry Tachycardia (AVNRT) – This condition is similar to AVRT, but the electrical signals here pass through the AV-node.
Atrial Flutter – The condition is the result of an irregular conduction circuit in the atria causing it to beat very fast. It can prevent blood from being pushed into the ventricles and prevent blood flow to the body cells.
Atrial Ectopic Tachycardia – Caused as a result of the electrical impulse being originated from a different part (an abnormal cluster of cells) than its usual origin (sinus node), it can result in the untimely contraction of the atrium and a faster heartbeat.
Junctional Ectopic Tachycardia – In this case, the impulse originates from the abnormal cluster of cells that are located near the AV-node, causing the ventricles to contract before they are due. This results in a faster heartbeat.
There is also a term called Sinus tachycardia which is a natural increase in the heart rate caused by physical activity or change in activity levels. Here the sinus node, which is known as the pacemaker of the heart due to its function of generating electrical impulses, shoots out faster impulses causing a spike in the heart rate.
2. Bradycardia
Bradycardia is characterised by a drastic drop in the heart rate of the child (in most cases, below 50 beats per minute). This can be due to faulty electrical circuits or can indicate that the sinus node isn’t functioning in the supposed manner. In some cases, since the heart rate drops quite low, the heart will not be able to pump blood into the cells, causing a deficiency that may result in life-threatening situations.
Causes of an Irregular Heartbeat
Irregular heartbeat is caused by various reasons including heart conditions like congenital heart disease, chemical imbalances in the body, any kind of trauma to the chest, heart surgery, fever or infections. It can also be due to cardiomyopathy, which is the disease of the muscles of the heart.
Signs and Symptoms of an Abnormal Heart Rate
Recognising the signs and symptoms of an abnormal heart rate is important, especially for younger children who will not be able to communicate what they feel. 2. Holter Monitor
This is an ECG test that is done over a period of 24 hours or more. The ECG electrodes and attached to the chest of the child and the measurements are taken using a portable recorder. The child is encouraged to engage in normal daily activities. Care should be taken to keep the electrodes from getting wet for the duration of the test.
Holter monitoring includes two kinds of test:
Continuous monitoring where the ECG is being recorded for the entire duration.
Event monitoring where the ECG is recorded when an abnormal heart rate is detected.
3. Electrophysiology Study
In this invasive test, a catheter is inserted into the blood vessel that leads to the heart through the arms or the legs of the child. This helps in determining the origin of arrhythmia in the heart and also helps the doctor choose relevant treatments methods.
4. Tilt Table Test
Tilt table test is recommended for children who have instances of fainting that is caused by irregular heart rate. The test measures the variations in heart rate, oxygen supply, and blood pressure in the child during a change in his or her position, like sitting, lying down and standing.
Some doctors may also suggest a chest X-ray, MRI or blood tests for your child if required.
Treatment of Arrhythmia in Children
The treatment for arrhythmia in children is based on the age of the child, the type of arrhythmia, the symptoms, and the frequency of its occurrence. Some of the common courses of treatment include (1):
Medicines: Anti-arrhythmic medications are prescribed to the child based on the condition and the age to correct an abnormal heart rate.
Pacemakers: These devices mimic the electrical signals produced by the sinus node. They are surgically implanted into the child’s body (usually near the collarbone) to regulate the heart rate effectively.
Defibrillator: An implantable cardioverter-defibrillator (ICD) is inserted near the collarbone with wires running from it to the heart. When an abnormal heart rate is detected, the device restores it to a normal heart rate.
Catheter Ablation: The procedure involves the insertion of a catheter through a blood vessel that leads to the heart. It detects and narrows down the area of the heart where the irregularity is generated, and freezes or destroys those defective cells.
Surgery: If all other options do not give results, surgery is recommended where the child is put under anaesthesia, and the tissue causing the arrhythmia is surgically removed.

Bacterial meningitis can affect when children come in close contact with the person carrying the infection, inhale infected respiratory droplets in the air, or touch infected surfaces and then touch their nose, mouth or eyes. The illness has long-term complications in children, such as seizures, hearing disability, brain damage, and even death. Viral meningitis can spread through personal contact, while fungal meningitis can spread when children inhale infected fungal spores.
What Are the Types of Meningitis?
Meningitis is broadly classified into three types:
Bacterial Meningitis – It develops suddenly and is life-threatening. It is often caused by bacteria like Streptococcus pneumoniae, Haemophilus influenzae type B and Neisseria meningitidis. The bacteria responsible for causing this infection can spread through person-to-person contact and respiratory droplets in the air.
Viral Meningitis – It is less severe than bacterial or fungal meningitis. Most people suffering from this viral illness recover without serious complications within 7 to 10 days. Common viruses that cause mild infections are influenza, non-polio enteroviruses, and measles and mumps.
Fungal Meningitis – It is rare and chronic caused by the fungus present in the environment. Premature babies have a high risk of contracting Candida fungus, which can travel to the brain and cause meningitis.
Of all three types, bacterial meningitis cases are up to 80%, and it is the deadliest and most common disease and can be prevented by vaccinating against the pathogens responsible for causing it.
What Are the Types of Bacterial Meningitis?
Out of 80% of bacterial meningitis cases, the two major bacteria that cause meningitis are Streptococcus pneumoniae and Haemophilus influenzae which can be prevented by vaccination as per India’s routine immunization program.
The third most common cause is Meningococcal Disease caused by Neisseria meningitidis, it is a deadly disease and needs an immediate cure when infected. It can be prevented by vaccination.

Neisseria meningitidis – Meningitis caused by N. meningitidis is also called as Meningococcal meningitis, and it has a high death rate. It usually affects kids and young adults. Meningococcal meningitis has a rapid onset rate, and the survivors may face long-term neurological complications. The fatality rate due to meningococcal meningitis is as high as 50% when left untreated. This disease transmits faster in crowded areas.
Streptococcus pneumoniae –The most frequently observed bacterial meningitis type, and it accounts for ~80% of cases.
Haemophilus influenzae (Hib) – Meningitis due to Hib is observed in young children and accounts for ~14% of cases. The meningitis episodes have a peak incidence in 6-12 months of birth.

What Types of Meningitis Vaccinations Are Available for Children? What Is Their Schedule?
Below listed are the type of vaccinations & their respective schedules available for preventing bacterial meningitis in children according to health experts:
Meningococcal conjugate vaccination (MenACWY) – The use of meningococcal vaccination is recommended for the high-risk group of children, young students travelling abroad to visit or study purposes, and travellers to Hajj and Sub-Sahara Africa regions. This vaccination protects against meningitis caused by Neisseria meningitidis. Children aged 9 months – 2 years can take two doses, three months apart, while kids above 2 years should take a single dose.
Haemophilus influenzae type b (Hib) Vaccination – This vaccination protects against Haemophilus influenzae type b (Hib) bacteria, which causes bacterial meningitis among young children. Hib vaccine is a 4-dose schedule to be taken during the time of birth, and then 6 weeks, 10 weeks and 14 weeks apart.
Pneumococcal Vaccination – This vaccination protects against pneumococcus bacteria that causes meningitis in children, majorly Streptococcus pneumoniae. Pneumococcal vaccine is a 4-dose schedule to be given to kids at the age of 6 weeks, 10 weeks, 14 weeks, and a booster dose at 15-18 months.

Is Meningitis Vaccination Safe for Children? Are There Any Side Effects?
The meningitis vaccine is safe and generally well tolerated. It may cause slight headache, redness, swelling and mild fever at the sight of infection, which is commonly observed with all other vaccines too. Who Should Get the Meningitis Vaccination?

The recent cases of Mpox (Monkeypox) have raised concerns, particularly for vulnerable groups such as children and pregnant women. Mpox, a viral zoonotic disease that can spread between animals and humans, typically begins with flu-like symptoms and is followed by a rash that can cause severe discomfort. While most cases have been mild, children and pregnant women may face higher risks of complications due to their unique physiological conditions and developing immune systems. They must take appropriate precautions to avoid possible infections.
To know more about the mpox virus, we must understand what monkeypox is, how it spreads, and what treatment options are available.
What Is Monkeypox?
Monkeypox is a viral infection caused by the monkeypox virus, a member of the Orthopoxvirus genus. It is a highly contagious infection with symptoms similar to smallpox but clinically less severe. The virus is primarily hosted by primates and rodents, such as rats and squirrels, which can transmit the virus to other animals and human beings. The virus typically occurs in the tropical regions of central and west Africa. Unlike smallpox, monkeypox is rare, less painful, and self-limiting (6).

History of Monkeypox
The first case of monkeypox was reported in 1958, following two outbreaks of pox-like diseases in monkeys—hence the name. However, the first human case was reported in 1970 in the Democratic Republic of Congo during the smallpox infestation drive. Since then, this viral infection has been recorded in humans in neighbouring countries.
Why Did WHO Declare Monkeypox as an “Emergency of International Concern”?
The World Health Organisation (WHO) declared Monkeypox (mpox) a “Public Health Emergency of International Concern” (PHEIC) on August 14, 2024, due to a significant rise in cases, particularly in Africa, and the emergence of a more virulent strain. A PHEIC is the highest level of alert that WHO can issue, and it is designed to coordinate international responses to significant public health threats (7).
The latest outbreak has been driven primarily by a new strain of the virus, Clade 1b, spreading rapidly in the Democratic Republic of the Congo (DRC) and neighbouring countries. As of mid-August 2024, over 15,600 cases and 537 deaths have been reported this year in the DRC alone. This strain has shown a concerning shift in transmission patterns, with increasing evidence of sexual transmission (8). This change in transmission has raised alarms about the potential for the virus to spread more broadly across populations.
Symptoms of Monkeypox
The monkeypox symptoms are similar but milder than smallpox. It usually takes 6 to 13 days to develop mpox symptoms, but the incubation period can range from 5 to 21 days (5).
The symptoms begin with:
Fever
Chills
Muscle aches
Headache
Backache
Exhaustion
Swollen lymph nodes
Following 1-3 days of fever, the individual typically develops rashes and skin eruptions on the face, which then spread to other parts of the body. These rashes can affect various areas, including the palms, soles, oral mucous membranes, cornea, and genitalia. It’s important to note that monkeypox is a self-limiting infection, with symptoms usually subsiding within 2 to 4 weeks. However, it’s worth noting that the infection may be more severe in children with underlying immune deficiencies.
Since this infection is rare, in severe cases, the fatality rate of clade 1 is about 3.6%, while clade 2 has a fatality rate of less than 0.2% (13).
How Does Monkeypox Spread?
Monkeypox is highly contagious. There are several modes of this viral mpox transmission. An individual may contract the infection by contacting an animal, person, or object contaminated with the virus. The virus can enter the host through mucous membranes (eyes, nose, mouth), abrasions (broken skin), or the respiratory tract (10).
Animal-to-human transmission can happen through:
A bite or scratch of the infected animal
direct contact with body fluids or lesion material
Contact with the contaminated bedding of the infected animal
It can spread between humans through:
Sharing respiratory droplets of any individual infected with monkeypox, like coughing and sneezing.
Direct contact with the infected person’s body fluids, lesions, skin blisters, or scabs.
Indirect contact by touching the infected person’s clothing or bedding.
Complications Associated With Monkeypox
Complications associated with monkeypox can vary in severity. Here are some key ones:
Pneumonia: Although less common, monkeypox can lead to respiratory complications, including pneumonia, particularly in severe cases or individuals with weakened immune systems.
Encephalitis: In rare instances, monkeypox can cause brain inflammation, leading to neurological symptoms such as confusion, seizures, or changes in consciousness (11).
Keratitis: The infection can sometimes affect the eyes, leading to keratitis, which is inflammation of the cornea. If not treated promptly, this can potentially cause vision problems (12).
Dehydration: Monkeypox can cause gastrointestinal symptoms such as diarrhoea, contributing to dehydration and nutritional imbalances.
Balanitis: Monkeypox can cause lesions and sores in the genital area, leading to balanitis, which is inflammation of the glans penis.
Treatment for Monkeypox
According to the CDC, there is no safe, proven treatment for monkeypox currently. However, antivirals and vaccines approved for smallpox can be used to treat the symptoms of monkeypox. As this infection can spread through touch, if you happen to contract monkeypox, isolation is necessary (4).
Mpox Vaccination
Since smallpox and monkeypox share similar symptoms, vaccination approved against smallpox, including Imvanex, has demonstrated to effectively provide protection against monkeypox as well.
The World Health Organization (WHO) recommends the use of certain mpox vaccines. These include:
MVA-BN (also known as JYNNEOS): A newer, safer vaccine for both smallpox and mpox.
LC16: An attenuated smallpox vaccine used in some regions.
ACAM2000: An older smallpox vaccine, used when other options are unavailable.
Vaccination is recommended primarily for individuals at high risk, such as those who have had close contact with someone infected with mpox or belong to high-risk groups. Mass vaccination is not currently advised (1).
Monkeypox and Pregnancy
Pregnant women are considered to be at higher risk of severe outcomes from monkeypox (mpox). Although the overall incidence of mpox during pregnancy is low according to the CDC and WHO, pregnant women who contract mpox may experience more severe symptoms compared to non-pregnant individuals. The complications of mpox during pregnancy can include miscarriage, preterm birth, stillbirth, and vertical transmission (9).
How Pregnant Women Can Avoid Getting Mpox:
Avoid close contact with anyone known to have mpox or with suspected cases.
Regular hand washing with soap and water or using hand sanitiser containing at least 60% alcohol can help prevent the spread of the virus.
If contact with infected individuals or contaminated environments cannot be avoided, wearing protective clothing such as masks and gloves can reduce the risk of transmission.
Monkeypox and Children
According to the CDC and WHO, children, especially those under the age of 5, may be at higher risk for severe complications from mpox. Children with mpox may experience skin lesions, high fever, lymphadenopathy (swollen lymph nodes), and general malaise (2).
How to Protect Children from Monkeypox:
To reduce the risk of mpox in children, consider the following precautions:
Ensure that children avoid close contact with anyone diagnosed with or suspected to have mpox.
Teach children to wash their hands frequently with soap and water and use hand sanitiser when handwashing is not possible.
Discourage sharing personal items like towels, bedding, and clothing that could be contaminated.
Be aware of any signs of mpox in children, such as unexplained rashes, fever, or swollen lymph nodes, and seek medical advice if symptoms develop.
What Should Parents Know About This Virus?
As monkeypox continues to be a global health concern, staying informed and taking simple precautions can help keep your family safe. Here’s what parents should be aware of (3):
Keep track of any reports of monkeypox cases in your community.
If you have older children who are in relationships, ensure they are aware of monkeypox and understand how to protect themselves.
If there’s a local outbreak, consider limiting children’s participation in crowded events or activities that involve close physical contact.
What Can You Do to Prevent It?
Several measures can be undertaken to prevent the contraction of this infection.
Do’s
Practise good hand and body hygiene regularly when coming from outside.
Consume meat that is sourced with clean checks and thoroughly cooked.
Wear masks when going outside, as the respiratory transmission of monkeypox is an option.
Don’ts
Avoid direct contact with the animals that can easily harbour the virus in the areas where the infection is increasing.
Avoid direct contact with sick animals and indirect contact with their beddings or other objects.
Do not come in close contact with people who are sick and may have monkeypox symptoms. Also, don’t share their belongings.
Avoid bushmeat (meat of wild animals).
Are There Any Cases of Monkeypox Found in India?
There have been no new monkeypox cases in India as of now. The government has implemented measures, including enhanced surveillance at airports and borders, and has prepared labs for early diagnosis. While India has seen around 30 cases since 2022, the last reported case was in March 2024.
Is India at Risk of Monkeypox Outbreak?
India is currently considered to be at low risk for a widespread monkeypox outbreak, according to health officials. The World Health Organization (WHO) has declared monkeypox a global public health emergency, but Indian health authorities believe that the chances of a large outbreak in India are low. This assessment is based on the relatively low number of cases reported in the country so far and the proactive measures implemented to monitor and contain any potential spread.

Step-7: Brush for a total of two minutes every day and night and give each tooth attention.
Step-8: Brush the roof and tongue gently using back to front motions.
Step-9: At the end, ask your child to spit and gargle.

2. Loop Method
This method is also known as the “Circle Method.”
