there is a poossibility that your child is having nutritional deficiency which is causing such changes in the child so give pplenty of water and seasonal fruits and vegetables and meet your doctor who will tell you whether any health supplements in the form of iron calcium vitamin d or multi vitamin is required or not

I suggest you please consult your pediatrician about this matter because physical examination of child is important for proper diagnosis and treatment
please avoid all packed food and RO water
please give daily sun light exposure
