My baby is 4 months old, I saw his left leg jerk for 2 seconds and this is the 3rd time, same happened 2 months ago it happened while feeding, and after that, he fell asleep, then I tried to burp, and his head was also shaken. Is there anything to concern?

Could be so I would not advised to take it lightly you can consult your doctor ones regarding the same and they will be able to give appropriate help on the issue that is there and it would be more convenient for you to understand if there is a problem
