A. At 23 weeks and 5 days, you're well into your second trimester, and taking low molecular weight heparin (LMWH) is often recommended in pregnancy for conditions like thrombophilia, previous clotting issues, or certain complications. When to stop LMWH depends on your personal medical condition and your doctor's plan for delivery. But generally: If you're having a planned delivery (like a C-section or induced labor): LMWH is usually stopped 24 hours before the procedure to reduce bleeding risk. If you're going into spontaneous labor: Your doctor may switch you to a shorter-acting blood thinner closer to your due date or advise you to stop LMWH at the first sign of labor (like contractions or water breaking). Typically, LMWH is continued until about 36–37 weeks, and your care team adjusts your plan depending on your risk factors and delivery type. Important: Never stop LMWH on your own. Always follow your obstetrician’s or hematologist’s specific advice, as it’s tailored to your pregnancy and health condition. Let me know your reason for taking LMWH if you’d like a more personalized explanation.
