When my wife breastfeeds our baby, the nipple on the right side hurts. What should we do now?

Because of a nipple crack or is it because of some nerve related pain like internal pain she feels because both the things are possible so if it is a crack she can apply Boroline antiseptic cream otherwise if it is related pain or internal pain that she feels that she should consult her doctor once
