give only breastmilk till completion of six months and feed more frequently for adequate hydration.
apply almond oil in both ears before bath and at bedtime

Ear scratching, nose rubbing, is very common at this age. This can be due to teething.
If ur baby is active and comfortable otherwise, no pain or uneasiness while rubbing ear then no need to worry.
