Q.
my child is 4 years old .. her weight is 12kg ... I'm doing some Remedial to gain weight .... then also her weight is not increased..

As far as the child is active for their age & also eating healthy enough no worries on that some varaition is acceptable, so not to worry

Your child should be 16 kg in this age a child is little less behind the schedule weight gain as per the age so along with the food that you are giving you can give good amount of priorities and what remedies you are doing I’m very certain about it if they’re not helping you then you may as well check with your doctor once
