Most often it was associated with significant nipple pain with latch and a bright pink color, but now we know bacterial infections are more likely and recent research suggests thrush is an unlikely cause of nipple pain. With red dots, one of the first things I'd wonder about is a skin reaction. Have you been applying anything to your breasts? The main contributor to nipple pain is a shallow latch and if you are pumping, considering whether that could be a source of irritation is also worth exploring. Ultimately, thrush is a medical diagnosis so it is fantastic that you've already checked in with your baby's medical provider. If this remains a mystery, you could ask your medical provider for input as well.
