my baby boy is getting small white spots on back side of the neck does it cause any harm

Spreading it could be problem or deficiency of calcium because white spots are usually associated with dryness or deficiency of calcium but a lot of times other things are also associated with it but these two are most common to examine with your doctor in the first place
