My son is 2.5 years old and he had the same issue. We consulted many doctors and they were saying that it was common. It occurs when the baby pushes too hard. So the key is to not let that happen. For that you can alter his food habits and give him a laxative after consulting with a physician. Avoid bakery items altogether and include broccoli, carrot etc in his diet. But it would take time for the food habits to take effect. If it is serious and occurring frequently, please consult a doc and give the poor thing a laxative.
