See the fever doesn’t subside within 72 hours then it is a problem of concern I don’t know from how long the fever is there and when did you start with the medicine but if it doesn’t subside within 72 hours maximum please consult your doctor for further assistance and the white patch could be due to dryness or deficiency of calcium
