A. It can be concerning when your baby’s SGPT (ALT) and SGOT (AST) levels fluctuate, especially after experiencing jaundice. These enzymes are indicators of liver function, and their fluctuation can sometimes signal liver stress or ongoing recovery from a condition like neonatal jaundice. However, given that all other tests came back negative, it's reassuring that no major underlying condition has been detected.
Possible Reasons for Fluctuation:
1. Recovery from Jaundice:
After having jaundice, it’s common for liver enzymes to fluctuate as the liver heals. Neonatal jaundice is usually caused by high levels of bilirubin, and liver enzyme levels can fluctuate for a while before stabilizing.
2. Liver Maturation:
In infants, liver enzyme levels can be higher as the liver matures. It’s not uncommon for babies to have transiently elevated liver enzymes that normalize as they grow.
3. Infections or Illness:
Even minor infections, like viral colds, can cause temporary elevations in liver enzymes. If your baby had a recent illness, this might explain the temporary rise.
4. Medications:
If your baby was on any medication, including antibiotics, it could cause temporary liver enzyme fluctuations.
What to Do Next:
Close Monitoring: Continue regular follow-ups with your pediatrician or pediatric hepatologist to monitor SGPT and SGOT levels.
Liver-Friendly Diet: Though your baby is still very young, ensure he is getting good nutrition, which can support liver function. Breastfeeding or formula feeding should be continued based on your doctor’s recommendations.
No Immediate Need for Biopsy: Since the tests have been negative and the liver enzyme levels are fluctuating but not consistently elevated at very high levels, your doctor may not feel a liver biopsy is immediately necessary unless there are other concerning symptoms.
When to Be Concerned:
If your baby develops additional symptoms such as yellowing of the skin or eyes (jaundice), poor weight gain, vomiting, or unusual tiredness, you should consult your doctor right away.
Conclusion:
The fluctuation in liver enzymes can be part of the natural recovery process, especially after jaundice. Since your doctor has ruled out major issues with other tests, it may simply require regular monitoring. If you're still concerned or if there’s no improvement in future tests, discussing the possibility of further testing or a specialist referral with your pediatrician is a good idea.
