baby weight gain issue

sharanya bade Guardian of a 4 yr 1 m old boy

my little one is not gaining weight. giving breast milk, formula milk and feeding cerealac too but he is underweight only. by some suggestions I am feeding idly with milk but after eating ildy he is vomiting please suggest me what to do
