Two year old boy not even speaking a word which doctor to consult specifically ENT or speech therapist

consult with speech therapist.
There are numerous potential reasons for a speech delay/disorder and a professional will be able to lead you in the right direction.

Encourage your child to express and respond. If there is no response even after persistent efforts then consult a speech therapist. Give your child a well balanced healthy diet and keep hydrated.
