I think it’s high time you consulted the dermatologist and get the child evaluated properly.
It usually doesn’t happen so we need to know if there is any underlying issue

it could be reaction to food, fabric or cosmetics ,heat or sweating or Infection too
so please keep an eye on your child diet , avoid giving processed oily spicy food and citrus fruits for now.
keep your child well hydrated
choose Cotton clothes for your child
skip suspected cosmetics
apply coconut oil or Cetaphil lotion on itchy part.
keep your child in well vantileted room
give bath twice in a day
for scratches , apply pure aloe vera gel on it.
