In most of the cases doctors do recommend when it is not healing or not settling down with any treatment that they suggested and surgery is good to be done for your child so you can definitely go ahead with it as of now as per your doctors recommendation in 20 MM is little high soso yes there can be a possibility that surgery would be the last option
