if the consistency is hard and the child is having pain while passing stool it is considered as constipation.
give plenty of water and fibre diet to the child
it is important to give fibre diet from cereals pulses vegetables and fruits
avoid giving more than 400 ml milk edveryday and avoid giving packet and processed foods as well as biscuits and refined food
adding papaya , fennel seeds water , oats pear prunes pineapple banana kishmish to the regular diet will help in decreasing cconstipation.

are you asking for your 3+ year old child?
if yes then I would suggest to keep your child well hydrated at first
because usually constipation is caused by inadequate intake of fluids and low fiber diet
give juice of soaked Black raisins daily in morning which work as Natural laxative
