A friend got this injury from a bike. What is this? What can be done to help? The blood is too thick it's not coming out and is unable to clean it

A. please get surgeon opinion. any such thing needs direct check up and examination. that will help to treat. will need antibiotics too in to help better a d and prevent infection.
