4 months baby is having constipation for more than 5 days frequently

If baby pass stool without any discomfort that is considered as normal
No need to worry
Give adequate feeding
Try to keep head up position while feeding
Give adequate feeding
Burp well after every Feeding
Do tummy time and cycling movement of legs to relieve gas and constipation
