As Water for newborn baby should be avoided for atleast six months.
Then how it is okay to give formula milk which is given by mixing water?
or is it because we mix hot water to form a formula milk so it is safe?
do we need to boil water or warm water is fine for formula milk?

To prepare formula milk as medium is use to dissolve formula milk powder,, water is best, that's why their is a proper ratio we use to prepare formula milk. 
using well boiled water is important to avoid infection, during preparation water temp should be room temperature or like warm.. not warm or hot, or else formula will loss it's nutrients
