my baby is approx three months old but his weight is still 3.8kg and his birth weight was 2.5 and he started social smile today only one time he smiled so I should worry or not

Continue to exclusively breastfeed your baby well on demand for better health, nutrition, hydration and weight gain. Slowly your baby will be reaching on the milestone.
