hello.. yes you can even repeat after 4 hours if fever spike comes after 4 hours.. 
 keep temperature checking 6 hourly..
Fevers higher than 104° F. should be attended to immediately.
Dress your child lightly. Use a sheet instead of a blanket to cover her in bed.
Breastfeed or give your baby formula often to maintain hydration of body.
Give soft easy to digest diet.
maintain proper hygein.

now my dr. tell to not give both medicine together, but yesterday I gave it.

Hello mom 
No you should not. There should be a six or gap between the medicine and it is very important. And before giving medicine you have to check baby’s temperature. Even after continuing medicine baby is not getting relief then you should definitely consult with your paediatrician again.
