Hi, my baby boy who is 4 months 28 days on lifting him on our arms tend to bend his neck down, once you show some things he will lift up. Now he himself will rotate body and lie on his tummy and turn around by lifting his neck up then also his neck will fall after some time and on displaying some toys he will lift his neck.

one of the doctor says not to worry and you says it's to worry y so many dilemma in suggestions

normal head holding and neck holding should come by 4 months of age and if it is not proper it is important to meet your doctor for assessment of other growth milestones of the child and also give a regular massage on the back and neck for three or four times everyday

Give adequate feeding every two hours of intervals 
Burp well after every feeding 
Try to keep head up position while feeding 
Take healthy protein rich nutritious food fruits vegetables to increase your milk 
More sucking leads to more milk 
Take a good care of your baby
