Diaper rash prevention is very important where certain things should be kept in mind specially in winter season. 
Changing your baby's diaper immediately  after it gets soiled is the key to prevention of diaper rash
Air dry when possible
 •  A good company diaper like pampers/ babyhug/ mamy poko should be used and diaper free interval fin between is very important.
Use irritant free productsc
Use a good diaper rash cream before and after using diaper like easinappy cream .
Also apply coconut oil to make the skin moist.

U Should keep that area absolutely clean sometimes you do.and sometimes due to unhygienic situation also such pimples can come and sometimes due to heat or irritation or infection also it can happen if it is okay with you I am going to advise you to get a check with a gynaecologist once
