my 4 month old baby passing motion 5 to 6 times watery per day sometimes its like smelly what we have to do

Consulting your doctor would be the right thing to do because in this age where you mentioning your child is suffering with lose motions with bad smell there are two reasons basically associated one is stomach infection second could be teething in either of the cases a child can suffer with loose motions
