for perianal/perigenital / diaper rash if it is less then you can go for simple coconut oil as well as any rash cream which is available for diaper rash like
(rashfree / ezinapi / b4napi / zincal)
but if it is not improving then at night time you can add t bact cream in the perianal area which will help in decreasing the rash.

to prevent the diaper rash in the future kindly apply coconut oil before making the child wear the diaper
normally hardness of the stool is not seen when you are giving only breastfeeding to the child.
it is advisable to continue only breastfeeding and nothing else till completion of 6 months of age.
if still the hard stool continues you have to visit the doctor to find the cause.
but if you are giving formula milkx then there is a possibility that the milk is not properly digested by the child and you will have to change the brand of formula milk.
also do notx give any food to the child except milk at this age.
