my 4 year old son has red patches on the back side of his neck since birth.

Then they could be the birthmark that your child is having if there was a simple patches would have come out by now but I think it is a birthmark that your child is having so for further clarity you need to get a check with your paediatrician they can give you hundred percent clarity on this
