you have to clean the ears of the child only externally with cotton cloth. 
do not use anything like earbuds to clean the internal ear. 
this will only do more harm as wax will get impacted inside deep and will cause more problem. 
if you feel that the child is having some problems in the internal ear then kindlyd meet your doctor for examination.
if the child is having wax problem and not improving with drops you have to get it removed with the help of your ent doctor. 
if there isx pain you can give paracetamol to the child for pain

Are you sure it is because of your child is unable to hear properly or you feel your child you are a dirty because I don’t think so that’s the reason if your child is continuously putting fingers into both the year let me the child is having another issue it is not necessarily because of that.yet if you want you can check with an ENT doctor one time
